As an RN Case Manager, you will perform comprehensive assessments upon admission, utilize established tools to identify high-risk patients, and ensure they receive the necessary support for a successful discharge. You will collaborate with multidisciplinary teams to coordinate care at various levels, educate patients and caregivers, and manage transitional planning to prevent readmissions and improve patient outcomes.
Key Responsibilities
- Comprehensive Assessments: Conduct initial and ongoing assessments to identify patient needs and potential discharge planning requirements.
- Multidisciplinary Coordination: Facilitate care coordination within and between hospital levels, ensuring seamless transitions and continuity of care.
- Transitional Planning: Educate patients and caregivers, arrange aftercare, involve commercial payers, and address high-utilizer patient root causes.
- Communication: Share relevant patient information with healthcare team members, fostering collaborative care.
- Performance Improvement: Contribute to reducing length of stay, preventing readmissions, reducing denials, and enhancing patient satisfaction.
- Utilization Review: Conduct commercial payer reviews, determine dispositions, perform retro/self-audits, and review admissions.

Requirements
- Education: Bachelor’s Degree in Nursing (BSN) or a related field required.
- Licensure: Current RN licensure in the state of New York.
- Experience: Minimum of 5 years of clinical experience in ED, critical care, or med/surg settings within an acute care hospital.
- Case Management: Previous case management experience preferred.
- Knowledge: Familiarity with healthcare financial, regulatory, and payer issues; state, local, and federal programs; Milliman/InterQual criteria.
- Certifications: PRI/Screen certification preferred; C.C.M. (Certified Case Manager) certification preferred.
