New Jersey’s academic health center, Rutgers Biomedical and Health Sciences (RBHS) takes an integrated approach to educating students, providing clinical care, and conducting research, all with the goal of improving human health. Rutgers, The State University of New Jersey, is seeking a Nursing Care Coordinator for NJMS- Ambulatory Care/Infectious Diseases. The primary purpose of the Nursing Care Coordinator position is to coordinate and deliver specialized nursing care services. Working within a healthcare setting, you will be responsible for ensuring that patients receive timely, compassionate, and evidence-based care, while also addressing their unique physical, emotional, and social needs. This role involves collaboration with a multidisciplinary team to optimize patient outcomes and enhance the overall quality of care
Among the key duties of this position are the following:
- Performs triage and medical assessments within scope of practice.
- Conducts thorough assessments of patients living with and/or at risk for HIV/AIDS to determine their healthcare needs.
- Coordinates and monitors the delivery of medical treatments, medications, and interventions.
- Monitors and tracks patient progress, ensuring adherence to prescribed treatment plans.
- Provides education and counseling to patients and their families regarding HIV prevention, treatment options, and adherence to care plans.
- Offers counseling and emotional support to patients and their families, addressing psychological and social aspects of living with HIV.
