Patient Safety Event and Quality Care Activities
·Use clinical assessment skills to perform analyses of patient safety processes and systems.
·Act as a clinical resource to Patient Safety Specialists.
·Coordinates and facilitates the investigation, review, and disposition of patient safety event reports.
oProvides support of patient safety processes to assist leadership and organizational committees with information to improve patient safety and quality care.
oTrack patient safety event data to identify trends for reporting purposes and in order to take and suggest actions for improvements.
oDesigns and presents effective patient safety education programs and presentations, as needed.
·Provides input regarding clinical processes and systems.
·Participate in readiness tracer activities to include reporting, as indicated.
Root Cause Analyses Activities
·Coordinates, leads, creates reports for and tracks trends for root cause analyses (RCAs) and improvement efforts.
oAnalyze and code HFACS data for ongoing trending of RCAs and improvement efforts.
Education and Collaboration
·Works closely and effectively with stakeholders from all disciplines, such as nursing, pharmacy, and medical staff.
·Conducts and/or participates in special projects/audits as assigned or necessary.
·Provides education and collaboration on patient safety issues as indicated.
Other Duties as Assigned
Bachelor's or advanced degree from an accredited School of Physical Therapy, Occupational Therapy, Nursing, Molecular Genetic Technology, Medical Technology, Clinical Laboratory Science, or other healthcare specialty. Five years of experience in a clinical specialty. (BLS) or (CPR) certification and must have one of the following licenses/certifications that is applicable to clinical specialty: *Current State of Texas Professional Nursing license (RN) *(ASCP) Board of Certification (BOC) *(CHT) by the American Society for Histocompatibility & Immunogenetics (ASHI) *Current license to practice physical therapy in the state of Texas issued by Executive Council of Physical Therapy and Occupational Therapy Examiners. *Current license to practice Occupational Therapy in the state of Texas issued by Executive Council of Physical Therapy and Occupational Therapy Examiners. *(RRT); *Licensed by the Texas Medical Board (TMB) to practice Respiratory Care *(MT) by the American Society of Clinical Pathologist Board of Certification (BOC) *(M) by the American Society of Clinical Pathologist Board of Certification (BOC) *(CLS) by the American Society of Clinical Pathologist Board of Certification (BOC). *(CIC) by the Certification Board of Infection Control. *(LMSW) issued by the State of Texas *(LCSW) issued by the State of Texas *(CRCST) by the International Association of Healthcare Central Service Materials Management (IAHCSMM). *(CER) by the International Association of Healthcare Central Service Materials Management (IAHCSMM). *(CFER) by the Certification Board of Sterile Processing and Distribution, Inc.
