POSITION SUMMARY
This position requires monitoring and reaching out to patient to improve clinical performance measures. Running reports and researching data on patients with diseases and preventive care measures using Cerner, HealtheIntent, Business Objects and insurance portals. Operates incoming calls and outreach programs to schedule appointments. Monitors patients until exams are completed. Use Disease Registry Cerner and Insurance Portals to reach out to patients who have not been seen by a Primary Care Physician. Knowledgeable on all new Providers to the Medical Group acts as a Physician Referral Service to all patients who do not have an established PCP and focuses on marketing of new Providers.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Runs monthly reports of patients that need to be contacted for Disease Management and Preventative Health compliance and report out on quality metrics for multi-specialty.
Updates Cerner system with the data from Cerner and community physicians.
Runs recall report on all patients who have not followed up on appointments.
Schedule/Track/ report out appointments to Manager of Population Health for multi- specialty.
Conducts outreach to patients with outstanding preventive care measures and disease measures; schedules appointments needed and records the appointment in the Cerner System.
Monitors documentation of appointments in Cerner to follow up if appointment is missed.
Monitors Patient Experience surveys by collecting documentation, and reporting out.
Conduct chart audits to ensure proper documentation.

Works with Manager of Population Health to prioritize the needs of the Medical Group.
Will not use or disclose individually identifiable health information of patients over and above that which is minimally required to perform your assigned position responsibilities.
Traveling as needed.
Other duties as assigned.
A. Education
Bachelor’s degree required in relevant subject area such as health administration, public health, nursing, social work, business administration, or related field. Candidates with at least three (3) years of experience in Population Health will be considered in lieu of degree.
B. Qualifications/Experience
6 months of customer service experience in a healthcare setting or similar.
CA Driver’s License required with proof of vehicle insurance.
C. Special Skills/Knowledge
Ability to read and speak Spanish fluently.
Ability to maintain a professional and patient care oriented attitude.
Must be able to multitask and perform each essential duty satisfactorily.
A team player that can follow a system and protocol to achieve a common goal.
Highly organized and well developed oral and written communication skills.
Confidence to communicate and outreach to other community health care organizations and personnel.
Demonstrates sound judgment, decision making and problem solving skills.
