Clerical Assistant
Performs secretarial and general office work as a member of the care management team to help facilitate the coordination of care, discharge planning, and post-discharge care. Provides regulatory compliance in the delivery of the Important Message to all Medicare beneficiaries and assists in the execution of the appeal process when necessary. Arranges post-discharge follow- up appointments to meet quality performance metrics of 89% of eligible discharges having a scheduled appointment and meets with patients to educate patient regarding appointments and identify barriers to go to appointments. Arranges patient transportation as indicated. Assists in the development, coordination, and implementation of projects to ensure the efficient operation of the department and participates in Katas for performance improvement.
Education – Graduation from high school or the equivalent required. Associate Degree preferred.
Experience – Previous clerical experience in healthcare environment preferred. Experience with office equipment to include Microsoft Office, facsimile, and electronic medical record experience preferred.
Licensure/Certification – N/A
Knowledge/Skills/Abilities – Effective communication and computer skills required. Knowledge of community resources preferred. Basic understanding of medical terminology required. Ability to prioritize work and multi-task to achieve desired results required.
