Position Summary
- Educates inpatients and families/significant others about diabetes including self care, survival skills and community resources in order for patients to effectively manage diabetes after discharge.Collaborate with all members of the healthcare team, including endocrinologists as needed. Accountable for education of the inpatient Nursing staff. This position maintains the standards of JCAHO, DOH, OSHA, EMTALA and appropriate certification requirements. Supports the mission, values, philosophy and objectives of HMC and practices within the framework of Shared Governance.
Primary Position Responsibilities
1. Diabetes Education - Provides education to inpatients and families regarding behavior objectives and strategies, new diabetes medical equipment and lifestyle changes. Discusses diabetes life saving skills.Trains inpatient on injection dosing and technique.
2. Documentation of Care - Completes documentation in EHR within 24 hours with evaluation of patient understanding, feedback and return demonstration noted.
3. Diabetes Technology - trains diabetic patients with diabetes technology if required. Feedback and return demonstration again noted.
4. Billing - Responsible for billing daily for services provided
5. Development of Diabetes Education Materials - Provides educational content and support to the inpatient. Shares expertise and collaborates with HMC staff on an ongoing basis. Provides education and assists with onboarding of new staff.
6. Maintains Patient Safety - Follows best practice guidelines. Stays current with the ADA guidelines.
7. Discharge Planning - Facilitates referrals from Providers to the Center for Nutrition and Diabetes Management, the Endocrine Practices or Primary Care for continuation of care.
Minimum Education Requirements
Required: Bachelor's Degree
Preferred: Master's Degree
Minimum Years of Experience
Required: 2 years of Med Surgical experience required
Preferred: Diabeters education experience
License, Registry or Certification
Required: RN, BSN (required to enroll and complete BSN within 5 years of position) Diabetes Education Certification required by 8 months after meeting clinical hours required, AHA BLS
Preferred: MSN, Diabetes Education Certificate upon hire
