From facilitating some of the most difficult conversations to addressing fears, hopes, and wishes with sensitivity and understanding, we play a pivotal role in guiding individuals and families through this transition.
Our team is dedicated to creating meaningful and lasting memories through unique end-of-life programs, such as the Gift of a Day initiative. Additionally, we collaborate closely with an interdisciplinary care team to ensure that each family and patient receives the necessary resources for comfort and well-being.

Social Work Qualifications:
Licensed Social Worker
Master’s or Bachelor’s degree completed in a program accredited by the Council of Social Work Education
2 years of experience working in a medical setting with experience with hospice and/or palliative care
Ability to work in both hospice & palliative programs
Provide emotional, social, and grief support to patients and family members, collaborating with spiritual and bereavement support staff.
Participate in Interdisciplinary Group meetings, identifying social-psychological issues and their interrelatedness with medical problems.
Assist in coordination and participation in Ultimate Gift Programs, such as Gift of a Day and Veteran Recognitions.
Reliable personal vehicle/transportation, valid driver’s license, and ability to safely operate said vehicle to visit patients and perform job duties within the field.
Social Work Benefits & Schedule:
Shift: Monday to Friday 8:30 AM to 5:00 PM with support for the on-call team.
