SUMMARY
Under the supervision of the Director, plans, advocates, implements, and coordinates a proactive array of comprehensive, individualized services for patients and performs administrative work in support of the program.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Conducts level of care assessments
Oversee and assign duties to Recovery Support Staff
Develop an individualized service treatment/recovery plan based on assessment data
Meets with patients regularly to identify needs and progress
Assist patients with the identification of appropriate program substance abuse, mental health, medical, and housing services in the city or town that they plan to live.
Assist patients with identification of vocational/educational/training resources.
Assist patients with job search to include resume writing, interviewing skills, appropriate dress, etc.
Conduct psycho educational groups.
Assist patients with paperwork to obtain program benefits and maintain required records (i.e., social security cards, birth certificates, physicals, lab work, etc.).
Establish and maintain required records to include progress and group notes, social security cards, birth certificates, physicals, lab work, and other appropriate records required by Phoenix House (PH) and outside agencies.
Attend staff meetings, case conferences and required training to coordinate with the treatment team and ensure continuum of care in accordance with clinical policy.
Maintain working relationship with agencies, resources and families to coordinate services for client care; provide information on substance abuse treatment resources and services; make necessary referrals; facilitate follow-up to ensure appropriate services have been delivered; and maintain aftercare follow-up system.
Establish and maintain client files in accordance with internal, local, state and federal requirements and enter data in computerized and manual admission and discharge systems.
Assist with quality assurance quarterly reports (i.e., fire drills, in-service trainings, safety inspections, satisfaction surveys, building and vehicle reports, etc.).
Serve as a member of the Case Management team for the review of prospective patients, for case conferences and for discharge planning.
Conduct and document follow-up for all patients on caseload.
Conduct Toxicology screens
Serve as a role model.

Qualifications
REQUIRED EDUCATION
LICENSES/CERTIFICATIONS
- Credentialed Alcoholism and Substance Abuse Counselor – Trainee (CASAC) in good standing
REQUIRED EXPERIENCE
Minimum two years’ experience working in a substance use disorder treatment setting, including working with patients with co-occurring disorders
Experience with case management and knowledge of and/or experience substance abuse, mental health, health, vocational/educational/training, and housing systems
WORK ENVIRONMENT
- This job operates in an indoor office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
