Job Description
Support HWD by managing Assisted Living team members.
Schedule Assisted Living team members.
Participate in Care Plan meetings with families and residents and manage ISP’s as resident needs change.
Promote all company policies by ensuring SLC communities are providing the best care possible for our residents.
Participate in weekly at risk meetings with HWD, ED, MCC/ MCD to ensure leadership team is aware of all resident needs.
Review and implement all necessary processes for optimal operational efficiency such as- shower schedules, laundry schedules, caregiver assignment sheets, etc.
Qualifications
Level of Formal Education: HS or GED Area of Study: General Years of Experience: 3-5+ years caregiver experience preferred Type of Experience: CAN experience preferred, scheduling, communicating with families Special Certifications: Prefer CNA Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties. Technical Competencies: Basic computer skills, Excel for scheduling Skills and Ability:
Need to be organized, flexible, and even tempered.
Ability to communicate with families, frontline team, and community management team to ensure optimal operational success.
Personal Attributes: Calm personality that possess the ability handle stressful situations with an easy demeanor, ability to multi task and follow through, and self-motivated.
