Overview Responsible for persons who have a substance abuse problem. Assess inpatient and outpatient treatment needs, identify interventions needed and refer patients for a broad range of services including methadone maintenance, detoxification, day treatment, residential treatment, and 12-step programs. Responsibilities Administrative Responsibility: Assists the Program Administrator in establishing direction and setting policies on substance abuse issues and in creating routine procedures and helps to develop programs. Case Finding: Works with all clinical departments to identify appropriate cases meeting program criteria for services. Psychosocial Assessment and Screens: Interviews patients to make an assessment of their need for program services and other referrals. Referral: Assures patient is referred to appropriate substance abuse treatment. Counseling: Conducts individual or group counseling sessions as appropriate, including pre and post-test HIV Counseling, follow-up counseling, etc. Case Management: Consults on the substance abuse issues of individual patients and assures that substance abuse treatment is integrated into the case management plan. Works in conjunction with social worker and case manager. Documentation: In addition to routine clinical documentation, assumes responsibility for the provision of accurate reports to MHRA. Liaison and “Linkage” Development: Develops relationships with outside agencies and groups, especially drug treatment program. Helps to forge direct linkages with appropriate providers and referral sources. Participates in Grant preparation and other duties as assigned. Qualifications Education: BA/BS Degree. Experience: Minimum three (3) years in substance abuse or addictive counseling related work. Knowledge and Skills: Familiar with community based and institutional substance abuse providers to ensure patients access to services. Ability to fully utilize BHMC’s electronic record systems, equipment and other healthcare and billing systems relevant to this position. Licenses, Certifications: License for or eligible for a New York State (C.A.C.) Credentialed Alcoholism Counselor and/or Credentialed Substance Abuse Counselor (CSAC). Physical Requirements: Position requires prolonged periods of standing, reaching, walking throughout the working day. Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
