JOB OVERVIEW:
The Eligibility Specialist is responsible for the day-to-day operations of the PA State Rebate funded transitional housing program’s application process, and initial review of all electronic and paper applications for the “Arms Around You” program. This position will also involve engagement with all subrecipients in a collaborative manner to assign participants to the appropriate subrecipient for services. This employee will work closely with the DHH Program Services Unit and subrecipient staff (Medical Case Managers, Housing Case Managers, and other subrecipient employees). In addition, the Housing Program Eligibility Specialist will work with external shareholders and collaborators (Office of Homeless Services, Division of Housing and Community Development, and external evaluators from the University of Pennsylvania).
RESPONSIBILITIES:
Access all applications in the RedCap system for admission.
Review and manage all incoming applications to access completion and eligibility for the transitional housing program.
Review accompanying documentation for accuracy.
Communicate with the applicant/referral source when documentation is missing from the application.
Conduct telephone interviews with applicants as needed.
Assign eligible applicants to a subrecipient for services.
Manage waitlists for the program
Work closely with the Transitional Housing Coordinator to ensure compliance with all applications and subrecipients.
Engage in collaboration with external shareholders and subrecipients.
Document all data pertaining to referrals.
Collaborate with internal and external colleagues.
Maintain inner office responsibilities.
Perform all other duties as requested.

EXPERIENCE:
Experience in engagement with participants/applicants

Preferred: Familiarity with the Ryan White HIV/AIDS Program preferred
EDUCATION: Bachelor’s degree from an accredited college or equivalent experience
