Acute Long-Term Care Hospital

An exciting and rewarding RN opportunity in a professional hospital setting assisting patients transition to inpatient rehabilitation, skilled nursing, and home care. Grace Hospital is a not-for-profit registered Acute Long-Term Care Hospital, the first of its kind to be established in Cuyahoga County. We are Medicare and Medicaid certified and fully accredited by the Joint Commission. Grace Hospital utilizes an interdisciplinary care team model to produce optimal clinical outcomes. We seek caregivers who are compassionate, competent, and results-oriented to join our team.

Specific Duties:

1. Oversees care provided by staff nurses, clinical therapists, patient care associates, nurse secretaries, and allied care providers.
2. Assists staff nurses as needed with direct patient care, including assisting with admissions and covering emergent staffing needs.
3. Develops and implements measures to achieve excellent clinical outcomes and high patient satisfaction scores.
4. Rounds on patients on a daily basis to assure needs are being met and to identify and resolve any open issues.
5. Works with staff individually and as a team to achieve high morale and low employee turnover.
6. Works directly with Nurse Educator to assure learning needs of clinical staff are identified and met.
7. Assists the Chief Nursing Officer with clinical and administrative projects as may be assigned.
8. Assists in the collection of data for quality, education, and infection control purposes.
9. Completes preliminary investigation of any medication variances and employee incident reports.
10. Completes all required education and competency requirements for the staff nurse position.
11. Flexes work hours to provide periodic evaluation and training for staff on all shifts.
12. Performs other duties as assigned.

Daily Duties:

1. Round on each patient daily to determine and meet individual needs.
2. Observe staff and assure all patient care activities are properly performed.
3. Review medical documentation to assure completeness and accuracy.
4. Provide leadership in meeting and exceeding clinical benchmarks.
5. Assist coordinate and problem-solve interdisciplinary clinical activities.
6. Counsel staff on clinical practices to sustain and enhance quality care.
7. Assure needed facilities, equipment, and supplies are readily available.
8. Initiate corrective action when performance requirements are not met.
9. Assist in presenting orientation, competencies, and other education.
10. Ensure each patient or family member completes a satisfaction survey.

Education: Graduated from an accredited school of nursing. Bachelor's of Nursing Degree.

Experience: Previous management experience required; 2-3 years of critical care experience preferred

Other: Licensed as a Registered Nurse in the State of Ohio; demonstrated leadership ability; professional demeanor in

communication, appearance, attitude, and performance; possession of a degree of creativity and latitude.
