Primary Duties and Responsibilities

As the Assistant Director for the UCLA Health Social Drivers of Health Program, you’ll help leadership shape the program’s direction and drive evidence-based screenings and interventions. By using data-driven insights to inform decision-making, you’ll spearhead innovation within the program. You’ll manage a Community-Based Organization network by establishing strong relationships with community partners, coordinating outreach efforts, and overseeing the integration of community resources into the program’s initiatives. The role also involves enhancing efficiency within the program’s projects.

You will:

Collaborate with the Director and Medical Director to establish program priorities and advance UCLA Health’s efforts to address social drivers of health and promote health equity.
Oversee the day-to-day operation of the program, coordinate cross-functional team, and work closely with other departments.
Collaborate with subject matter experts, healthcare professionals, and community partners to design evidence-based interventions that address identified social needs and risks.
Establish key performance indicators for the program and individual projects, tracking progress against these metrics and benchmarks.
Contribute to the development of evaluation frameworks to assess the impact of social drivers of health interventions and identify areas for enhancement.
Develop a community network of community-based organizations and agencies to support patients’ social needs.
Ensure the program's adherence to and reporting of relevant standards, regulations, and reporting requirements to accrediting bodies, health plans, and government agencies.
Foster a positive, inclusive work environment that promotes collaboration, creativity, and accountability.

Salary Range: $78,800 - $175,000 Annually

Job Qualifications

We’re seeking a collaborative, passionate, caring professional with:

Bachelor’s degree required. MPH, MHA, MBA, MSW, or other related Master’s degree preferred
Licensed Clinical Social Worker preferred
Minimum seven years of program leadership/management experience in health care or public health
Prior experience in a social impact area, including directly or indirectly leading cross-functional teams preferred
Proven understanding of social care and social drivers/determinants of health
Experience in leading, facilitating, and implementing new programs and/or projects
Excellent communication/presentation skills
Proficiency with Microsoft Office
Prior experience supervising employees, including selection, training, work assignments and review, motivation, and performance appraisal
Skill in working with complex organizations to comply with regulatory and health plan requirements
Ability to interact diplomatically and empathetically with a patient population in varying degrees of health and effective communication with staff, faculty, and the general public in conveying information to varying organizational levels
