We are seeking a Lead Care Manager join our team at our program in Sacramento, CA.

Position Details

Join our team as an Enhance Care Management (ECM) Lead Care Manager (LCM) and play a pivotal role in providing comprehensive case management services for the California Advancing and Innovating Medi-Cal (CalAIM) initiative. Responsibilities include:

• Collaborating with clients to develop care management plans addressing their diverse needs.
• Conducting comprehensive assessments and managing care coordination.
• Promoting health and wellness through outreach and health education.
• Facilitating transitional care and referrals to community services.
• Identifying and addressing client support needs across various domains.

This role involves a dynamic mix of outreach, assessment, coordination, and support services to ensure holistic care for our clients.

Earn up to $28.00/hour

The ideal candidate will possess the following qualifications:

• Bachelor’s degree in the social service field.
• Bilingual in Spanish preferred.
• 1 year experience in care coordination/case management required.
• Valid CA Driver’s License required. Travel is necessary to offer support service within the community.
