INTAKE/ASSESSMENT PROCESS Assertively responds to telephone and/or walk-in requests for psychiatric assistance in a timely manner. Assesses each inquiry to determine clinical need of caller and/or patient, and is assertive in offering in-house or mobile assessment in order to transition patients to the appropriate level of care. Identifies high risks, i.e. suicidality, homicidality, potential for fall, Medical problems, etc., and documents clearly on Intake Screening Form, completes High Risk Screening Form and hands off both to unit staff. Enters all calls requesting outpatient and inpatient services into the AS400 Database daily. Conducts daily follow-up on all pending cases. Conducts follow-up in any way necessary to provide assistance until there is a disposition. Has knowledge of and identifies exclusionary criteria and review with Intake or Nursing Supervisor. Documents all inquiries on Intake Screening Form, including requests for beds that are currently not available. Conducts follow-up on potential bed availability REFERRAL PROCESS Maintains a good working relationship and ongoing communications with referral sources. Contacts Therapist, Nurse, Parents, or other Person(s) to obtain pertinent medical and clinical information in order to thoroughly complete the Intake Screening Form. Telephones direct outside referral source regarding disposition of case. Has good knowledge of Intake Referral List and is able to identify other health or human service agencies that may be helpful to the patient. Familiar with Kaiser Helpline and assists outside referrals to obtain authorization to admit Kaiser Permanente patients to Del Amo Hospital. ADMISSIONS PROCESS Knowledgeable of admission criteria for all services, units, and programs. Offers admission to Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) levels of care when appropriate. Obtains signatures for “Conditions of Admissions” from adult patients admitted on involuntary detention (5150). Knowledge of exclusionary medical criteria/parameters and reviews with Intake staff, Nursing staff or Internal Medical staff. Documents on all call sheet issues that require medical review and clearance. Cognizant of Adult, Youth and Indigent physician rotation; carefully designates and documents physician assignments. Inquires as to whether potential patient has access to dangerous items that can potentially be used to harm self or others has been assessed. Carefully screens all Detox admissions for severity of withdrawal symptoms, quantity and frequency of use, treatment history, etc. Provides report including High Risk Form to Unit Nurse. Ascertains that there are orders from physician on the unit. ADMISSIONS PROCESS: Financial Information Researches insurance benefits carefully to determine correct payer sources (i.e. Medicare vs. Medicare HMO, Medi-Cal vs. Commercial plan), as well as correct telephone number for pre-certification. Ensures that an attempt is made to pre-certify/authorize patient benefits with Commercial and Medicare HMO by end of shift. Knowledgeable of Midas Systems and enters pre-certification/authorization notes into system. Careful to obtain Kaiser Permanente Psychiatrist Authorization for all Kaiser Permanente patients. Experience: Previous experience in a clinical department of psychiatric hospital and previous clinical experience with Psychiatric patients of all ages is preferred.
