Who We Serve

Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.

JOB SUMMARY

Works within the vision, mission and philosophy of the agency. Creates a strength- based relationship with youth and assures a positive environment. Assists in the design and implementation of interventions and strategies in support of the youth and family’s goals. Ensures the provision of safety and health in a variety of environments for youth and families in accordance with agency standards. Coaches primary care givers and other community members.

RESPONSIBILITIES AND DUTIES

• Provides supervision and living environment support for youth in residential settings.
• Builds solid, cooperative, culturally responsive relationships with youth and families as well as community resources to assist with the planning, organizing, implementation and evaluation of appropriate activities to achieve family identified goals.
• Builds on individual strengths, concerns, and needs with balanced focus on the family as a unit to assist in the implementation and achievement of goals and outcomes.
• Works with youth and family teams to support family driven, strength-based planning and interventions.
• Conducts observations regarding specific target behavior, track progress and modifies plans with the family to support their intended identified outcome.
• Identifies behavioral triggers and suggests environmental modifications to support positive behavioral change.
• Provides targeted mental health services consistent with service plans, documents in alignment with medical regulations and achieves set productivity expectations.
• Provides and documents other non-billable services and activities as required.
• Ensures the safety of and is accountable for the whereabouts of youth at all times.
• Assists in developing program responses to needs identified across a number of youth and families where natural community resources are not available or appropriate.
• Facilitates, supports, and models participation in healthy group dynamics within various settings including family homes, schools, parks and recreation centers, and treatment facilities; and provides an open forum for expression of feelings and ideas when appropriate.
• Understands and takes advantage of therapeutic opportunities in crisis situations.
• Monitors effectiveness and efficiency of behavioral and crisis management interventions with youth.
• Participates in physical restraints using Therapeutic Crisis Intervention techniques.
• Required to meet applicable productivity and documentation requirements.

Other Duties and Responsibilities

• Practices self-care remains aware that others may be contending with stress and treats other with grace.
• Driving to appointments with clients and their families or other key contacts.
• Physical interactions with children of variable ages such as but not limited to games, sports, and other activities.
• Performs other related responsibilities, as assigned, to support specific department/business needs.

Education Requirements

High School Diploma or GED required.

PREFERRED EXPERIENCE

One (1) year of related experience with youth in a residential or related setting.

Other Specific Requirements

• Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Child Abuse Index Check (CAIC) background clearance.
• Valid California Driver's license with two years' experience driving and clean driving record may be required.
