1. JOB SUMMARY
The Nurse Practitioner is a Masters prepared registered nurse, who possesses additional educational preparation and skill in physical diagnosis, psychosocial assessment, and management of health illness needs. This individual works in collaboration with the medical and hospital staff to provide multidisciplinary care of patients across the continuum of care and assists in the development, implementation, and evaluation of outcomes for the Los Alamitos Medical Center patient population. This position brings a cohesive element to care, providing comprehensive care management, overseeing patient and family teaching, and consulting with members of the healthcare team to ensure all aspects of patients care. These elements are accomplished efficiently; provide clinical management and education for patients; utilize a scientific approach in the analysis of patient care; identifying opportunities for quality improvement, cost reduction, and re-education based on current research findings. The position practices in accordance with TJC State and Specialty regulatory criteria and practices under the guidelines of standardized procedure as authorized in Title 16, Section 1474 of the Nursing Practice Act.

Knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and management of chronic patient/family problems in the acute care and outpatient settings.

Nurse Practitioner Functions: (as outlined in the LAMC Nurse Practitioners approved Standardized Procedure)
1. Obtains history and performs physical examinations of the patient pertinent to the chief complaint.
2. Manages the care of the patient, collaborating with the physician when necessary according to LAMC approved standardized procedures.
3. Asses the status of the patient and intervenes with appropriate orders and interventions. Monitors patient progress to ensure clinical and financial outcomes.
4. Orders and evaluates appropriate diagnostic studies.
5. Orders appropriate therapeutic interventions.
6. Furnishes drugs and devices within scope of practice under Standardized Procedure.
7. Assesses, implements, and manages follow up plans
8. Records and documents data, interventions, results, patient education and outcomes.
9. Advises and educates patients and families concerning health related problems. Participates in patient/family support activities. 10. Initiates appropriate referrals for patients.
Mentors, develops, inspires, and leads staff members in patient and procedural settings, maintaining and promoting standards of care and developmental of critical thinking skills and role modeling advanced practice.
Accountable for clinical and financial patient outcomes where appropriate. Collaborates with patient, family, physicians, and health care providers to achieve optimal patient care management and outcomes.
Assists in tracking and trending outcome performance data on patient populations. Assist in the maintenance of databases specific to assigned patient populations. Participates in cost quality analysis making recommendations. Develops and implements action plan for practice changes to enhance patient outcomes.

MANDATORY:
Education: Masters of Science Degree in Nursing
Experience: 3 years as a registered nurse

Current CA RN License
Current CA Nurse Practitioner Certification
CA Nurse Practitioner Furnishing Certificate Current National Provider Identification
Current DEA License at time of credentialing
MSN graduates from 2008-present are required to have and maintain National Board Certification as a Nurse Practitioner for the patient population being served.
Current AHA BLS Certification
Current ACLS Certification
Current NIHSS Certification
PREFERRED:
Education: Doctorate Nursing Practice Degree
Experience: 5 Years Experience as a Nurse Practitioner
Licensure/Certification:
Sub-specialty certification in practice area.
