Job Summary
The Office of Diversion and Reentry (ODR), within the Los Angeles County Department of Health Services, develops and implements programs to divert people with serious mental, physical and/or behavioral health needs away from the LA County Jail and into community-based care.
The Case Manager II works as a team member with social service professional and paraprofessional staff ensuring the delivery of quality care in accordance with the rules and regulations established as well as adhering to the mission of St. Joseph Center.
ESSENTIAL DUTIES & RESPONSIBILITIES
The Case Manager II key areas of responsibility include:
Key Areas of Responsibility:
Develop and maintain strong ties to the court systems, community, and law enforcement.
Link clients to appropriate bridge/interim housing programs and assist them in completing required paperwork in order to obtain appropriate permanent housing.
Develop and maintain strong collaborative relationships with other service providers involved in the client’s care.
In the absence of program manager, Case Manager II with assist with day to day operations of ODR team
Provide crisis support/screening to other case managers on ODR team.
Maintain appointment scheduling of psychiatric appointments for clients.
Essential Duties:
Attend monthly clinical meetings with the ODR team and SJC team.
Lead weekly team check-in meeting. Lead weekly team meeting in absence of program manager.
Participate in case conferencing meetings, clinical consultation meetings, and other community meetings.
Respond to referral assignments within program expectations. Initiate and complete necessary tasks prior to client’s release date.
Provide case management and supportive services to meet the needs of each individual.
Build trust and rapport with individuals exiting/recently exiting the jail system. Assess homeless individuals utilizing the Vulnerability Index (VI) SPDAT.
Link clients to physical health, mental health, and other supportive services. Assist clients to enroll in mainstream benefits and obtain identification (i.e. California Driver’s License, Social Security Card).
Keep highly organized files for each client and enter data and progress notes into the appropriate electronic databases.
Ensure court progress report and all other documentation is completed in a timely manner.
Provide bus tokens, bus passes, taxi vouchers, and/or direct transportation as needed.
Participate in case conferencing meetings and other community meetings.
Coordinate with other outreach teams in joint outreach efforts.
Ensure timely and regular meetings with clients in jail settings, street outreach locations, client homes and/or homeless encampments.

Qualifications
Knowledge, Skills & Abilities:
Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent organizational skills and the capability to work in a fast paced environment.
Strong knowledge of services and resources. Demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, and Housing First.
A bilingual background is a plus.
The position requires an ability to work flexible hours including some early mornings and evenings.
Experience:
Two years’ experience providing services to highly vulnerable populations.
Education:
Bachelor’s degree from an accredited college or university in Psychology, Social Work, Human Services or a related field.
Direct Reports:
This position has no direct supervisory responsibilities.
Computer Skills:
Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
Ability to use widely supported internet browsers.
Ability to use electronic health records and other various electronic data bases.
Certificates, Licenses and Registrations:
Valid California Class C Driver License.
Valid automobile liability insurance.
Travel Requirements:
Travel to client sessions, meetings at multiple St. Joseph Center sites, and at community partners’ locations as needed.

Salary: $24-26/hr
