Resource & Referral Health Outreach Specialist (Community Health Outreach Specialist)
What We're Looking For:
We are seeking a Health Outreach Specialist who will assist in delivering program services to eligible families and the general public through multiple health care options such as Medi-Cal, My Health L.A., and others. The Health Outreach Specialist is responsible for coordinating outreach, enrollment, utilization, retention, advocacy, and referral activities. All duties and responsibilities are performed in accordance with contractual obligations under the different programs, as well as agency and department standards. The ideal candidate will also have the following responsibilities:
- Perform outreach and enrollment activities, and explain the Medi-Cal Program as well as other health care options to child care providers, parents, and the general public.
- Assist families in enrolling, utilizing and retaining health care options, including scheduling appointments.
- Develop new contacts for outreach activities through networking activities; encourage Medi-Cal, Healthy Families transition and other health insurance consumers to share their positive experiences with other community members.
- Maintain records, logs, sign-in sheets and other documentation required to track compliance and evaluate performance.
- Enter client data into the Children's Health Outreach Initiative (CHOI) database.
- Work with appropriate Department of Public Health staff and other relevant organizations/individuals to coordinate information dissemination and other services.
- Represent the agency and advocate programs at professional organizations and community events; participate in work groups, committees and ad hoc groups.
- Working in the field and with minimal supervision, the employee independently plans, organizes and prioritizes work following established procedures and guidelines; must have the ability to work flexible days as work may include evenings and weekends.
EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES YOU SHOULD POSSESS:
- Bachelor of Science/Arts degree from an accredited university in Health Education, Social Services or related field; relevant work experience in human services/health education/child development may be substituted for college level education. Must complete process to become a Certified Application Counselor(CAC) within the first 45 days in the position and/or possess a CAC certification.
- Bilingual skills English/Spanish in both oral and written communication preferred. Must have a minimum of two years of experience working with families receiving subsidized child care or other social support services
- Must have working knowledge of the Department of Public Health, Department of Public Social Services, the Safety Net Program and other community-based programs.
- Must maintain a valid and current California Drivers License; have use of a motor vehicle; and provide evidence of automobile liability insurance every six months.
- Must provide verification of valid TB clearance within the last year of employment.
- Must successfully pass the Department of Justice (DOJ) criminal background check within 90 days of employment.
