$2,500 Sign-On Bonus* The Fairfax-Falls Church Community Services Board seeks an energetic, highly organized, self-motivated wraparound facilitator. A wraparound facilitator is focused on developing and leading an intensive, individualized plan of care and safety plan for children and youth. The goal of the Youth and Family Team is to effectively and successfully meet the emotional and behavioral needs of the youth and family in order to eliminate the need for more restrictive placements. The wraparound facilitator works with children and youth who are at risk of placement in a residential treatment facility or are currently placed outside his/her home and are transitioning back to the community setting. The wraparound facilitator works intensely with the family, community partners and informal supports. The facilitator works in the community and will meet with families in their home on a weekly basis to identify and develop a collaborative Youth and Family Team tasked with creating, implementing and monitoring a sustainable strength-based individualized Plan of Care and Safety Plan. Will partner with the family to lead the Youth and Family Team and will introduce the team to the principles and practice of wraparound services to include the use of a team-based planning process. Will utilize/manage funds provide by the Office of Children's Services (CSA). Will monitor the progression, efficacy and on-going needs of the youth/family as identified in the Plan of Care and will be responsible for the development and distribution of all documentation relevant to the activities of the Youth and Family Team.

The CSB provides clinical supervision for employees who are registered as licensed eligible mental health in one of the following areas: Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Clinical Psychologist, Licensed Marriage and Family Therapist, Licensed Substance Abuse Treatment Practitioner or Clinical Nurse Specialist.

Illustrative Duties

Maintains a diverse caseload of clients with a variety of substance use, developmental disabilities and/or mental illnesses;
Develops client and/or family centered therapeutic treatment plans, with consideration given to the nature and severity of the behavioral health problem, family, cultural and employment related circumstances, legal involvement, individual and family histories, physical condition, and other pertinent factors;
Collaborates with service professionals inside and outside of the Community Services Board;
Adapts and modifies treatment interventions as indicated;
Provides case management, outreach and engagement services as needed;
Maintains all electronic health records according to federal, state and department regulations, ensuring that records of clients on their caseload are complete, accurate and comprehensive;
Advocates on behalf of clients regarding rights and needed services;
Develops and maintains cooperative relationships with representatives of community groups and public and private agencies;
Assesses and monitors the impact of medication on client's functioning and consults with medical professionals as needed;
Provides entry, navigation and/or referral services for individuals, families and other concerned persons;
Testifies in court to regarding client's services, progress and recommendations as required.
Required Knowledge Skills and Abilities

Knowledge of existing public and private agencies and community resources;
Ability to formulate diagnoses and appropriate treatment plan;

Employment Standards
MINIMUM QUALIFICATIONS: Graduation from an accredited college or university with a bachelor’s degree in psychology, social work, nursing, counseling, or a related field, plus 1,500 hours of clinical experience; OR a bachelor’s degree and a valid QMHP certification plus 1,500 hours of clinical experience; OR a master’s degree in psychology, social work, nursing, counseling, or a related field plus 500 hours of clinical experience.
CERTIFICATES AND LICENSES REQUIRED:
Valid Motor Vehicle Driver's license with fewer than six demerit points (or the equivalent in another State) required within 1 month of hire and maintained throughout employment with CSB.
Qualified Mental Health Professional Children and Adolescent (QMHP-C) - required within 1 month of hire.
NECESSARY SPECIAL REQUIREMENTS: The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.
This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)
PREFERRED QUALIFICATIONS:
Master's degree in social work, psychology, counseling or related field.
Experience working with Office of Children's Services (CSA).
Experience in leading and/or facilitation team-based planning meetings.
PHYSICAL REQUIREMENTS: Ability to fulfill assigned duties on site/at various locations (client home/school/juvenile court/other agencies). Ability to observe client behavior/appearance; converse with clients and co-workers; sit/walk/stand/bend/write as needed to complete; and independently drive a vehicle. All duties performed with or without reasonable accommodations.
