Under the direction of the Director/Clinical Manager, initiates the nursing progress and assumes responsibility for the plan of care for assigned patients. Provides care focused on the spiritual, physical, social and emotional needs identified. Assesses, plans, implements and evaluates the patient’s physical, psychological, and socio-cultural needs. Collaborate with other members of patient care team to achieve goals. Maintains knowledge of government regulations which affects home care reimbursement.
Qualifications - Minimum
Education:
2 years acute care experience required
Professional knowledge of nursing theory and practice at a level normally acquired through completion of an Associate’s degree in Nursing
Diploma in Nursing and one year’s current experience within the last five years in an acute care setting is necessary.
Approximately one to two years’ experience preferred in order to become fully skilled in assessing patient needs, implementing and evaluation effective plans of care and being knowledgeable with hospital and departmental policies and procedures.
Current license as a Registered Nurse in the state of Ohio. Maintains annual certification in CPR. Current, valid driver’s license in the state of Ohio.
Maintains automobile insurance coverage with limits of coverage at least 100K/300K liability. Ability to utilize homecare computer data system accurately.

Hours: 8:00am-8:00pm
