Requirements:
- Bachelors degree in social service related field (social work, psychology, counseling, criminal justice, sociology, or similar)
- 1-3 years of experience implementing case management services and documenting interactions
- Drivers license
- Ability to pass DOC security clearance
- CPR / Adult First Aid certification preferred
Job duties:
Facilitating excellent participant programming:
- Conducting impactful 1:1 and group sessions, developing a comprehensive understanding of every participant on caseload and ensuring treatment goals are met
- Facilitating group sessions focused on life skills such as money management, healthy sexuality, hygiene, self-care, and refusal skills
- ?Connecting participants to community resources?
Communicating with multi-disciplinary care team:
Documenting sessions (for individual and groups), including noteworthy participant interactions in electronic record system
Documenting Reflections using standard forms
Working collaboratively with the Community and Life Skills team, career coach, and therapist assigned to each participant, providing input on progress to ensure consistency in instruction/learning pre-to-post release and alignment with participants goals
Participating in internal meetings (Community and Life Skills team meetings, All Team meetings, 1:1 supervisor meetings, and training sessions)

Pay Details: $60,000.00 to $70,000.00 per year
