The Patient Care Coordinator/Clinical Research Registered Nurse is a hybrid position that provides patient care and case coordination to persons with HIV infection including pre and post test counseling, education of patient, family/significant others, staff and community. Assesses need for medical, social, psychological interventions and facilitates appropriate referrals. Established working relationships between community based service providers and hospitals to assure continuity of care. This position also encompasses duties which include subject recruitment and data collection for entry into a RedCap database. This position will provide educational materials to the recruited subjects.
Responsibilities
A day in the life of a Patient Care Coordinator/Clinical Research Registered Nurse at Hackensack Meridian Health includes:
Patient Care Coordination:
- Provides pre and post test counseling for clients regarding HIV testing/results.
- Responsibilities include:
- Write and submit completed annual application for Ryan White Grant.
- Coordinate and provide necessary documentation for reimbursement of clinic services through grant.
- Oversight of fund management and electronic communication monthly in compliance with the federal and state regulations for the Ryan White grant.
- Attend mandatory meetings for Ryan White Grant.
- Coordinating and submits reports for the federal research project Special Projects of National Significance (SPNS).
- Track monthly expenses and reimbursement for services provided under the grant.
- Provide education and materials for clients, family, significant others regarding HIV infection, AIDS, as the situation requires.
- Provides educational programs/materials as requested for hospital staff and community (World AIDs Day).
- Develops individual care plan for clients in coordination with others involved in care (physician, social worker, nurse).
- Assess the need for dietary counseling, social services, financial assistance, support group, etc. Arrange referrals through the appropriate channels.
- Coordinates arrangements for home care, meals-on-wheels and other community agencies. Initiates referrals for entitlements as appropriate.
- Maintains expertise as AIDs coordinator through participation in appropriate continuing education programs.
- Member of the Infection Control Antibiotic Committee
- Facilitates working relationship between non-hospital and hospitals providers to assure continuity of care.
- Ensures that patient confidentiality is maintained.
Clinical Research:
- Assures that all protocol revisions, informed consents, continuing reviews, serious adverse events are submitted to the appropriate Institutional Review Board (IRB) of record in a timely manner.
- Interacts with regulatory specialists and principal investigators on all regulatory issues and changes within the protocol.
- Reviews study with principal investigator and/or clinical research coordinator, and clinical team, participates in the review of studies for feasibility and evaluates potential competition with other protocols prior to submitting study.
- Recruits and evaluates potential study patients, and works with clinical research coordinator to a budget outlining standard of care and research costs. Finalizes budget draft with budget coordinator.
- Recruits and evaluates potential study patients, and works with a clinical research coordinator to schedule required appointments and interviews.
- Identifies the needs of the patient population served and modifies the delivers care that is specific to those needs( ie. age, culture, hearing and/or visually impaired etc). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy etc)
- Reviews medical records for potential study patients and ensures that medical records include documentation of all laboratory test results and procedures and progress of study patients, following guidelines set forth by the protocol sponsors.
- Educates study patients concerning informed consent procedures, HIPPA authorization.
- Documents study patients medical history including but not limited to past medical/surgical treatments, significant medical conditions and medication history per protocol guidelines.
- Evaluates and develops study patient education materials and gives study patient and/or designated caregiver instructions on drug administration and other medical information; creates study specific calendars for study patients.
- Notifies principal investigator of any adverse events and serious adverse events, including evidence of drug toxicity or unexpected side effects.
- Maintains accurate, complete, up-to-date records on each patient participating in a clinical study protocol.
- Develops case report forms and/or databases for physician initiated studies as needed.
- Assists the principal investigator in preparing for publication. Works with analysts and assists with queries related to data to evaluate the significance of collected data.
- Provides education to all departments and clinical areas where study is performed.
- Attends research meetings and conferences as required.
- Participates in staff meetings and in-service education of nursing and medical staff.
- Other duties and/or projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
- BSN required or BSN in progress and/or willing to acquire within three (3) years of hire
- Minimum of 3 years nursing or social work experience.
- Completion of a fellowship in endocrinology in a recognized institution.
- Experience in endocrinology/diabetes and/or an affiliation with an academic institution
- Adheres to the American Nurses Association standards.
- Strong attention to detail and customer service focus is required.
- Excellent verbal and written communication skills.
- Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
- Ability to relate effectively to all levels of the clinical and administrative structure of the medical center as well as of other institutions affiliated with the Division of Endocrinology.
Education, Knowledge, Skills and Abilities Preferred:
- HIV antibody testing and counseling training.
Licenses and Certifications Required:
- NJ State Professional Registered Nurse License.
- BLS/CPR certification by the American Heart Association is required within 60 days of hire.
