The schedule for this position is Monday - Friday, 8am - 5pm. This is a field-based position visiting our members in their homes, community based housing, and long-term, skilled and acute care facilities in the Washington DC area. You'll need to be flexible, adaptable and, above all, patient in all types of situations.
Primary Responsibilities:
- Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
- Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:
- Master's degree in Social Work or other related clinical field
- Active and unrestricted LICSW or LGSW license in Washington DC
- 2+ years of experience in long-term care, home health, hospice, public health or assisted living
- 2+ years of experience working with MS Word, Excel and Outlook
- 1+ years of clinical or case management experience
- Access to reliable transportation
- Ability to work in a field-based capacity in Washington, DC
Preferred Qualifications:
- Experience with electronic charting
- Experience with arranging community resources
- Field-based work experience
- Experience working with elderly/geriatric patients
- Background in managing populations with complex medical or behavioral needs
