Our New Horizons Intensive Care Coordination team is committed to providing competent, creative high-fidelity wraparound services that improve and empower our youth and their families to successfully manage complex mental health, substance use and co-occurring disorder issues with community-based services. We are seeking qualified applicants to fill a full-time Family Support Partner (Clinical Services Caseworker Associate) position in the New Horizons Intensive Care Coordination Program. This is a SAMHSA Grant funded position that goes through 9/29/2027.
About This Role: New Horizons Intensive Care Coordination (ICC) Program offers a team environment and flexible work hours. In this role you will:
Provide leadership at all levels in the creation and maintenance of a positive and collaborative working environment where all Intensive Care Coordination (ICC) staff and enrolled youth and their families feel respected and supported.
Use personal past experiences, provide peer support to parents/caregivers served in the ICC program.
Assist parents/caregivers with successful engagement, advocacy, and connection to formal and informal resources helping them to understand high-fidelity wraparound (HFW) process/services and build resiliency as a family.
Provide effective peer support utilizing the HFW framework service to parents/caregivers.
Work collaboratively with ICC to provide high-fidelity wraparound (HFW) care for clients and families.
Utilize the HFW framework, conduct comprehensive assessments as needed.
Develop and implement peer support treatment plans to promote progress of client and family.
Timely clinical documentation into EHR that meets requisite agency, Federal, Medicaid, MCO and licensure standards.
Administer to all youth and families being served related data tools completed at identified data collection points.
Attend youth and family team meetings. Coach parents/caregivers and family members on how to advocate for themselves.
Document of all services, contacts and attempted contact with client, their family and collaterals.
Use social media or other forms of communication to engage families and provide them guidance, information, support and resources.
Maintain healthy boundaries with clients and their families.
Attend all mandatory county and state trainings on ICC and HFW Support Partner Training, to include refresher trainings. Participate in individual and group coaching, as required.
Carry an after-hours work phone and provide telephone and, at times, face-to-face after-hours support to youth and families.
Minimum Requirements:
High School diploma or GED equivalent
Must be the biological or adoptive parent or person in the parent role who has been the primary caregiver of a child with emotional, behavioral, or mental health challenges
Must have experience navigating the Virginia behavioral health systems with their child
Preferences:
5 years of experience providing services to youth and families with complex mental health, substance use and co-occurring disorders
1-2 years of experience providing recovery services in a human services environment
Experience with and participating in HFW intervention or in family partnership meetings
Experience collaborating with families, natural supports, and other community stakeholders
Certification or registration or considering certification or registration as a Peer Recovery Specialist or National Certified Peer Specialist
Experience assessing client needs and providing access to appropriate services
Experience providing clinical documentation necessary to maintain client records including treatment planning/wellness recovery plan, progress notes, quarterly reviews and annual assessments
Be a proficient user level of MS Suite and an EHR
Bachelor’s degree in psychology, social work, counseling or a related behavioral science field
Bilingual and multilingual candidates encouraged to apply
Special Requirements:
Must complete the HFW Support Partner Training
Must possess a valid driver's license
Must pass DMV record check
Must pass TB test,
Must pass and maintain first aid and CPR certification
Offer of employment is contingent upon the candidate passing a fingerprint based National Criminal History Record Check,Child Protective Services Requirements: Effective 1999, mandated by the Department of Behavioral Health and Developmental Services 12VA35-105-400 rules and Regulations for Licensing Providers by the Department of Behavioral Health and Development Services related to Criminal Registry Checks, a search of the registry of founded complaints of child abuse and neglected maintained by the Department of Social Services.
Work Schedule: This full-time position, based in our Woodbridge office, requires a 4 or 5-day work week that includes evening hours. Starting Salary Range:$70,921.50 - $97,968.00
