The HealthySteps Program at the ZSFG Children’s Health Center is seeking to hire a full time, non-clinical patient navigator to support connection for families with young children to community-based services that promote optimal health and wellbeing.The position involves providing non-clinical support, guidance and assistance for families with children ages birth to five years old as they navigate through complex healthcare environments and early childhood network of care in the community. Primary goals are to promote child and family health and wellness by tailoring navigation supports to each family’s unique needs, in hopes of facilitating more timely linkage and reduce missed opportunities for linkage due to complex systems of care and other social determinants of health. Common family navigation needs include (a) supporting connection for children with developmental needs to Early Intervention evaluation and services through Golden Gate Regional Center, (b) support for caregivers to navigate to and enroll in their own healthcare services, such as postpartum healthcare or mental health services, (c) connection to culturally-concordant, community-based health and social resources to promote wellness and mitigate race-based health inequities and social determinants, and (d) linkage with case management services to support families with multiple social and health-related needs. Acts as a communication liaison to understand families’ non-clinical needs, desires, and concerns. Guides pediatric patients and their families to a broad range of services, amenities, and information to promote healing and ensure satisfaction with the patient care experience.

Assists with navigating all aspects of the healthcare system, including mitigating the siloing that often occurs between the hospital-based medical system and community providers and services. Uses knowledge of the unit or clinic to resolve issues and identify ways to increase family satisfaction. Collaborates with the CHC HealthySteps team and department managers to meet families’ expectations, achieve quality outcomes and build a family focused culture. Recognizes and upholds values of diversity, equity, and inclusion.
This position will work in partnership with the multidisciplinary CHC HealthySteps team, including health workers and navigation volunteers, licensed clinical social workers, licensed marriage and family therapists, clinical psychologists, pediatricians, HealthySteps interns other allied health professionals. The HealthySteps team is part of a rich multi-departmental initiative crossing the SF Department of Public Health, UCSF Pediatrics, and UCSF Psychiatry to promote whole person and whole family care and in order to set babies and toddlers up for optimal wellness by providing early interventions and support for families as early as possible.
We are especially interested in candidates who identify with the communities we serve, have bilingual capacity in English and Spanish, and with experience working as a community health worker or navigator in a county public health system. Familiarity with the resources available to families with young children and within the early childhood network of care in San Francisco is also desirable. A passion to advocate for an underresourced population is required. The HealthySteps family navigator will join a mission-driven interdisciplinary pediatric health care team, and will have opportunities to participate in all Children’s Health Center offerings, including staff meetings, trainings, supervision, and bi-annual wellbeing retreats. The family navigator position will also join a growing navigation service of navigators across a variety of departments, such as Solid Start and Team Lily, and who are dedicated to meeting the needs of pregnant individuals/expecting families and families with young children who are served within the public health system of San Francisco.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $55,200 - $95,900 (Annual Rate).

- Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Strong knowledge of data collection, compilation, and analytical techniques.
- Strong skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. In-depth knowledge of the organization and how to get issues resolved.
- Strong knowledge of the early childhood network of care and ability to tailor appropriate supports for families depending on needs that arise within the early childhood network of care

- Fluency in Spanish language
- Bachelor's degree in related area and minimum three years of experience or equivalent experience / training.

- Strong interpersonal and customer service skills. Ability to communicate and resolve issues effectively with a diverse population of patients, staff and physicians.
- Excellent analytical and problem-solving skills. Ability to develop solutions and recommend changes and follow through with implementation.
- Excellent written and verbal communication skills.
- Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.
- Ability and willingness to engage in individual and group-based reflective practice to support optimal patient outcomes and team functioning.
