The Home Health Clinical Manager provides ongoing guidance and direction to the multidisciplinary team.
Essential Functions:
Ensures the provision of quality service by assigned staff. Provides ongoing direction, guidance and evaluation of clinical staff and ensures that professional and Agency standards are maintained. Assesses requests for services and initiate measures to provide for meeting identified service needs.
Primary Duties:
Evaluates the level of competency and general professional performance of their staff and conducts field supervisory visits.
Identifies the in-service educational needs of staff individually and collectively and participates in planning and implementing appropriate orientation and in-service programs.
Directs and guides staff in caseload management and establishing priorities.
Directs and guides staff in their professional development in performing job responsibilities.
Ensures documentation is current, accurate, and in keeping with Agency policies.
Ensures effective interdisciplinary communication and care coordination to promote attainment of positive patient outcomes.
Analyzes the referral needs to ensure provision of appropriate and adequate service and to determine staffing needs.
Reviews clinical documentation and ensures timely completion of workflow to facilitate the billing process.
Demonstrates understanding of key drivers for home health revenue and the level of impact to Agency financial performance.
Meets qualitative and quantitative performance measures established by Agency.
Establishes and maintains effective working relationships with staff, peers, leadership team, and community agencies.
Interprets home health services and programs to patients, families, community, and other agencies.
Builds and maintains relationships with referral sources in collaboration with the Business Development team.
Maintains clinical skills and professional competencies.
Maintains, improves, and applies knowledge of principles, practices, and techniques of management.
Assumes responsibility for professional growth and continuing education.
Participates in the educational process of students.
Interprets Agency service & programs to patients, families, community, and other agencies.
Participates in Quality Assurance Activities.
Knowledge and Experience:
Registered Nurse with minimum of three to five years clinical experience, including supervisory or management responsibilities.
Prior experience in a Medicare-certified home health agency.
Knowledge of CMS (Centers for Medicaid and Medicare Services) home health Conditions of Participation and other regulatory requirements for home health.
Excellent verbal and written communication skills.
Exceptional critical thinking skills.
Highly organized, self-directed, and motivated individual with ability to foster employee engagement and retention.
Ability to effectively prioritize and multitask in a fast-paced work environment.
Computer proficiency including proficiency with EMR system.
Current RN License.
Education: Graduate of an accredited school of nursing with a diploma, Associates degree or BSN.
