Compensation: $135,000-$150,000

We are seeking an experienced Nurse Practitioner to join our team, focusing on member engagement and care management for high-risk populations, with an emphasis on black and brown communities. The role involves assessing healthcare needs, developing patient action plans, managing care transitions, and coordinating care for patients with complex, chronic medical and mental health conditions.

Primary Responsibilities:

• Develop and use care pathway templates by chronic condition.

• Deploy remote patient monitoring and patient self-reporting tools.

• Conduct in-home or tele-health assessments and respond in real-time to patient alerts.

• Provide care coordination, patient navigation, chronic disease management, and interdisciplinary collaboration.

• Engage patients in managing their health, medications, treatments, and follow-up appointments.

Qualifications:

• Active California state Registered Nursing license and BSN required.

• At least 5 years of relevant clinical experience, with 3+ years in care management preferred.

• Excellent communication skills and proficiency in Microsoft Office.

• Ability to build and manage relationships effectively.

• Commitment to creating an inclusive work environment and addressing care gaps for black and brown populations.
