The Neuroscience Nurse Navigator coordinates all services required to provide multidisciplinary, quality assured, appropriate case selection and cost-effective care to neuro patients. The Neuro Nurse Navigator serves as a resource for patients, physicians and others serving in the role of educator, quality improvement and consultant to patients referred. Makes initial contact with the patient/family to establish a professional relationship and understanding of the diagnosis, treatment, and follow-up regimen. The nurse assesses the patient’s status and works with physicians to determine appropriate plan for patient and assess appropriateness per regulatory and vendor guidelines. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
Provides coordination and lateral integration for the Neuro patient between clinicians, management and patient care activities to achieve high standards of patient care and quality outcomes.
Works with the multidisciplinary team including physicians, staff, internal and external partners to ensure patient's progress along the continuum of care in an efficient and cost effective manner that ensures quality outcomes.
Utilizes established evidence based guidelines to support appropriate level of care throughout the hospitalization and beyond.
Acts as a patient advocate and communication link with other departments, and community resources.
Position Requirements:
Required: Bachelors Degree
Preferred: Masters Degree
Specify Degree(s): Nursing- Minimum
Experience:
Required: 3-5 years of previous job-related experience
Preferred: 6-10 years of previous job-related experience
Licensure/Certifications:
Required: CPR (Cardiopulmonary Resuscitation) & Current Registered Nurse License State of Virginia.
Neuroscience or Stroke certification required within one (1) year of placement into role.
NIH Stroke Scale Certification required within 90 days of placement into role.
Preferred: Specialty certification in clinical or functional area of nursing granted by a national nursing organization i.e. SCRN or CNRN
