Position Overview: The Case Manager is responsible for providing assistance to the licensed clinician in support of the Maternal Mental Health Program by assessing the clients' wellness needs and developing a care plan. This program intends to expand mental health resource navigation by providing access to comprehensive mental health services to individuals navigating the challenges of pregnancy and parenthood from pre-conception to postpartum periods. The case manager is accountable for ensuring delivery of exceptional services that are focused to the client's needs.
Duties & Responsibilities:
Assess for participant's needs by reviewing and collaborating with Case Managers and Interns on mental health screenings and needs identification to address physical, emotional and mental wellbeing
Engage with clients and their supportive networks including but not limited to healthcare providers to achieve progress towards care plan goals
Facilitate individual and group therapy sessions as needed with program participants and maintain care plan progress of each participant, initiate referrals to community mental health resources
Build and maintain trusted relationships with program participants, partners, stakeholders, and administrators
Collaborate with community providers to provide a safe space and environment for mental health counseling for program participants and their families
Maintain weekly scheduled appointments in the electronic medical records (EMR) system.
Communicate participant information and needs with the supervisor, program manager, coordinator, case manager, and other staff as needed
Evaluate and record clients’ overall program progress, including assessments, home visits, progress notes, referrals and all follow-up interactions in the EMR
Conducts outreach activities to inform community groups/agencies about Maternal Mental Health Program services

Requirements
Education:
Master's degree in Social Work, Psychology, Public Health, Health Education, or related field
Experience:
Two (2) years of experience providing direct mental health services
One (1) year providing trauma-informed counseling services
One (1) year providing services to high-risk vulnerable populations
Licenses/Certifications:
Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC)
Valid Driver’s License and auto insurance (Texas upon hire)
Knowledge, Skills and Abilities:
Maintain necessary licensure and certifications required to practice in the State of Texas.
Experienced in trauma-informed care
Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)
Excellent communication skills (verbal & written)
Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
Ability to maintain confidentiality when dealing with sensitive information
Ability to work well with public and staff members

Full Time | Grant Funded
Monday – Friday | 8 am – 5 pm
Must be willing to flex work hours when required, including weekends.
Expected to travel within Harris County jurisdiction.
Work Environment:
This job operates in a professional office environment with occasional visits needed to private residential homes, clinics, and other community organizations.
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands:
This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
