About the position:
The Patient Care Coordinator is responsible for patient scheduling, patient contact, and primary referrals made outside of the clinic. The PCC oversees administrative/non-medical operations in the clinic.
Key Outcomes:
In the first 90 days, the PCC will:
Learn and live the Americares values.
Complete all required new hire onboarding trainings.
Demonstrate understanding of the free clinic model.
Complete Learning Modules for Athena.
Demonstrate understanding of basic communication skills and medical terminology with patients, learns proper phone etiquette to answer phones.
Learn and perform scheduling and referral processes.
Learn and perform procedures for patient check-in and enter demographics in Athena.
Learn and perform screening process to determine patient eligibility.
Learn procedure and begin to schedule patient appointments and referrals.
Shadow experienced interpreter to learn how to properly interpret. Review and verbalizes understanding of safety plan for clinics.
In the first 6 months, the PCC will:
Demonstrate proficiency with workflow for patient scheduling and referral processes in Athena EHR.
Demonstrate proficiency with workflow for patient screening process in Athena EHR.
Interpret skillfully and demonstrate proper patient interpretation skills.
Demonstrate ability to handle all patient calls, understanding which patients require medical intervention.
Demonstrate ability to manage patient referrals accurately and schedules appointments.
Learns and demonstrates process for Patient Assistance Program (PAP).
Provide follow up for complicated referrals and knowledge of community resources
In the first year, the PCC will:
Confidently and skillfully handle all patient inquiries both phone and in person.
Demonstrate mastery of all procedures for scheduling and referrals.
Demonstrate mastery of Athena EHR; demonstrated ability to document and update patient charts.
Proficiency in PAP program procedures.
Develop relationships with community agencies to facilitate meeting patient needs outside of health (food, housing, etc.)
Duties and Responsibilities:
Schedule patient appointments both on and off the clinic site. Provide patient records to the referral source and advise the patient of the details of the follow-up visit.
Maintain follow-up log to track patient referrals, obtain reports, and requested medical records.
Manage all aspects of Patient Assistance Program.
Oversee screening process including training new volunteers to obtain proof of income and residency to determine eligibility for clinic services and PAP.
Respond to patient/telephone inquiries regarding clinic hours and services.
These competencies are required for the role:
High school graduate or equivalent
Demonstrated administrative experience in a medical office or clinic
Fluency in English and Spanish
Familiarity with medical terminology
Strong interpersonal skills and ability to work well with medical staff, volunteers, and patients.
Demonstrated ability to prioritize workflow and work independently yet respond to requests from others as needed.
Demonstrated experience in a busy, team setting with a team orientation and balance of personal accountability and team goals.
US work authorization is required for this role.

Fluency in languages that are spoken in at least one of our core locations (India, Philippines, Tanzania, El Salvador, Puerto Rico, Colombia, etc.), in addition to English and Spanish.
