Job Duties and Responsibilities: Responsible for psychosocial assessments of the patient’s mental health and drug and alcohol history; individual, group, and family therapy; active participation on the treatment team; lead role in the development of the master treatment plan. Developing realistic and measurable short- and long-term goals; and assumes responsibility for coordinating with other agencies involved with the patient the discharge and continuing care plans.
