Key Responsibilities:

• Direct, coordinate, and supervise the operations of the Department of Case Management.
• Monitor service activities and resource utilization, taking appropriate action as necessary.
• Prepare and manage the annual departmental budget, ensuring alignment with hospital guidelines.
• Develop and implement departmental goals, policies, and procedures.
• Manage human resources functions, including staff selection, promotion, orientation, and performance appraisals.
• Ensure compliance with hospital standards and regulatory requirements, directing quality improvement activities.
• Coordinate discharge planning processes and ensure continuity of care with related departments.
• Maintain effective relationships with community service organizations and public agencies.
• Provide educational programs for hospital staff and students to enhance understanding of patient care.

Requirements:

• Master’s Degree in Nursing (MSN) required.
• Minimum of three (3+) years of progressive case management leadership experience in an acute care setting.
• Current New York State Registered Nurse License.
