JOB SUMMARY:
The Care Coach, a nationally certified medical assistant, engages directly with patients through one-on-one encounters and group sessions. Their primary role is to support patients in managing their chronic conditions and achieving their established care plan goals.
DUTIES AND RESPONSIBILITIES:
- Conduct face-to-face visits with patients to promote and reinforce established care plan goals, including education on lifestyle modifications.
- Collaborate with the Care Educator Nurse to discuss cases and ensure the closure of patient care gaps.
- Identify and attend to same-day patients who can benefit from participation in the care education program.
- Facilitate patient group workshops as coordinated by the Care Program Nurse Manager.
- Accurately document patient information within the Electronic Medical Record (EMR) system.
- Participate in health fairs and other events aimed at promoting health within the community.
- Fulfill any additional duties assigned by the Care Program Nurse Manager.
- National certification as a medical assistant.
- CPR/BLS required.
- Two years of medical assistant experience preferred.
- Bilingual (fluency in speaking Spanish) is preferred.
- Strong interpersonal skills and ability to engage effectively with patients.
- Knowledge of chronic condition management and lifestyle modification techniques.
- Proficiency in documenting patient information in an EMR system. Epic experience highly preferred.
- Ability to collaborate effectively within a multidisciplinary team.
- Flexibility to adapt to changing priorities and patient needs.
