The City of Pasadena Public Health Department is currently seeking an enthusiastic and skilled Community Services Representative III to serve in a “Family Navigator" role to support the California Innovations in Services and Partnerships for Inclusion, Resilience and Empowerment (Cal-InSPIRE) program. Cal-InSPIRE is designed to serve families of children with or at risk of developmental delays, disabilities, and other health care needs by using innovative strategies that break the mold, address broader needs of the child and their family, create systems-level change, and focus on strengths-based, resilience-building approaches. The Family Navigator’s main responsibility is to work collaboratively with the Maternal, Child and Adolescent Health (MCAH) Director and MCAH Coordinator in fulfilling the Family Navigator responsibilities outlined in the Cal-InSPIRE scope of work. The Family Navigator reports to the MCAH Coordinator.
IDEAL CANDIDATE The ideal candidate will possess a passion for helping others; knowledge of local, state and national agencies related to Individuals with Disabilities Education Act (IDEA) services; excellent oral and written communication skills and the ability to deliver professional presentations to families and service providers; strong organizational skills and exceptional attention to detail; ability to multitask and adapt quickly to change; ability to work with diverse populations; ability to develop and maintain collaborative relationships with staff members at partner organizations; a proven record of exercising good judgement and professionalism; and proficiency with traditional office software programs. The Cal-InSPIRE Family Navigator will build trust and communicate with families to provide emotional support, education, and advocacy, and will assist with problem solving to resolve immediate or recurring barriers to receiving services.
ABOUT THE DEPARTMENT Since 1892, the Pasadena Public Health Department has been responsible for helping protect, maintain, and improve the health of the Pasadena community. The City of Pasadena is one of only three cities in the State of California that maintains its own independent local health jurisdiction with responsibility for a wide variety of services that support the three core functions of assessment, policy development and assurance.

Essential Functions

Facilitate a collaborative workgroup of stakeholders focused on children and youth with special health care needs (CYSHCN).
Co-create solutions with families to improve CYSHCN-serving systems such as integrating leadership development opportunities, building advocacy skills among parents and promoting civic participation.
Create or update a resource guide or diagram to help families, providers, and organizations understand the landscape of available local resources in the community.
Engage families at schools, libraries, and community outreach events.
Develop and facilitate educational trainings and presentations to inform families, staff, service providers, community organizations, program partners and policy makers to increase awareness of community needs, barriers, and navigation services for CYSHCN and their families.
Offer rapid response (e.g., a phone call within 48 to 72 hours) when a family has requested assistance.
Listen to families’ concerns and provide confidential emotional support, coaching, and collaborative problem solving in one-on-one and group settings to resolve immediate or recurring challenges and barriers to receiving and/or participating in services or programs.
Actively participate in connecting families to services by supporting them to make phone calls, resolving transportation barriers, and providing information about services in multiple systems with the goal of client empowerment. This may involve education and advocacy as well as system navigation.
Plan and facilitate resilience-building activities for families to increase social connections, prioritize family bonding, cope with stress, and increase confidence to advocate for their children.
Support early childhood professionals, including librarians, in creating inclusive environments for children with special health care needs and their families.
Coordinate developmental screening for children enrolled in MCAH programs and in the community. Ensure staff know how and when screening occurs, what their role is, and how to use screening data to plan. Share the benefits and purpose of screening with families. At a team meeting with other staff, family, and mental health consultants or child development professionals (when appropriate), review a child's screening data and discuss the next steps. Share results with parents and provide referrals as needed.
Document contacts and outreach activities including logs, client files, and sign in sheets for the annual report.
Complete quarterly time studies including secondary documentation as required by California Department of Public Health (CDPH).
Assist with coordinating and implementing special projects and events; perform other duties as assigned.
Ability to travel locally as needed.

EDUCATION AND EXPERIENCE
Graduation from high school or GED equivalent.
Three years related experience in human services, early childhood education, special education, or related field.
PREFERRED QUALIFICATIONS
Completion of Early Childhood Development or Social Work units is desired.
Knowledge of local, state, and national agencies related to Individuals with Disabilities Education Act (IDEA) services and family supports including special education and regional center services.
One-year related experience working directly with parents and connecting families to local resources.
Ability to build trusting, effective working relationships with families and providers.
Excellent oral and written communication skills, interpersonal skills, and strong organizational skills.
Bilingual preferred.

SELECTION PROCESS The selection process will consist of a training and experience evaluation, oral exam, and department-level interview.
