Job Overview:
The Licensed Social Worker acts as a support system for people with drug and alcohol/mental health disorders and their behavioral health issues and maintains a case load that includes IOP levels of care. The LSW responsibilities include group facilitation, individual sessions, intakes, documentation, outreach and follow up.
The Union County Department of Human Services (UCDHS) partners to deliver Mental Health and Recovery Support Services in the areas of Intensive Case Management, Peer Support, & Community Base Case Management.
Qualifications:
Required:
- Master's degree in Social Work
- At least one year working experience as a LSW in a substance abuse program
- All new hires who are in recovery must attest to having one year of sobriety and abstinence
- All new hires are required to sign an Affidavit of Recovery
- Must comply with TRMC paid state level criminal history record background check supported by fingerprint check prior to hire
Preferred:
- Knowledge of ASAM criteria and experience with NJSAMS
- ACT certificate
Certifications and Licenses Required:
- NJ Licensed Social Worker
Scheduling Requirements:
Essential Functions:
- Teaching individuals how to modify behaviors
- Assessments including intake, identifying issues and creating goals and individualized Treatment Plans
- Assisting with case management
- Leading group therapy sessions
- Providing outreach and progress reports to referring agencies
- Referring patients to support groups
- Setting up an after care plan
- Obtaining authorizations from Medicaid, insurances and other referral sources
- Other duties as assigned
