We are seeking a highly motivated and experienced Program Manager to lead our community and school health initiative. The Program Coordinator is responsible for coordinating, supervising, implementing and evaluating programs, services and activities for the Center for Healthier Communities for multiple grant funded projects. These programs focus on areas including chronic disease management and prevention and abuse prevention programs. This position is also responsible for program development and grant writing. Responsibilities

Qualifications
Knowledge of local community agencies and resources.
Experience managing community based programs abuse prevention ability to create multi-disciplinary team grant reporting and project management.
Experience creating and presenting workshops and evidence based curriculums.
BS/BA degree required or 5 years’ experience in community based programs or social services.
Ability to travel locally for outreach events (some evenings and weekends).
