JOB DESCRIPTION AND POSITION REQUIREMENTS

JOB SUMMARY:
The Outpatient Clinical Pharmacist will assist in the development, implementation, and documentation of various outpatient pharmacy endeavors. Primarily, this individual will assist with patients enrolled in the system’s Anticoagulation Clinic, especially with patients utilizing vitamin k antagonist therapy. In addition, this individual will assist with processing prior authorizations to allow for optimum patient access and assist in the development and growth of the system’s pharmacy based medication refill center. This will be accomplished through collaboration with the Pharmacy Clinical Coordinator of Outpatient Services, Pharmacy AVP, clinical and staff pharmacists, medical staff, and other healthcare professionals. Performs all functions relating to the provision of information about dosing, side effects, and compatibilities for all patients ranging from neonates to geriatric.
MINIMUM QUALIFICATIONS:
Bachelor of Science in Pharmacy or Pharm. D degree. Must be licensed in the State of Louisiana. Obtain at least 15 ACPE approved education hours per year. Must have computer skills and dexterity required for data entry and retrieval of patient information. Must be proficient with Windows-style applications and keyboard.
Preferred Qualifications: Completion of PGY1 Pharmacy Practice Residency preferred. Five years related experience in specialty pharmacy preferred.
PHYSICAL DEMANDS:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise.
