Through screening mechanisms identifies patients who are at high risk for need of social services intervention. Provides assessments to identify bio-psychosocial emotional substance use maturational cultural adjustment financial and/or legal problems then intervenes by implementing suitable interventions. Facilitates linkage/assistance that enables patients/families to access medical care to achieve optimum levels of health and fulfill goals for treatment. Functions may also include discharge planning.One to three years experience in a medical facility with acute or outpatient psychiatric experience preferred. One to three years experience in an acute care setting as a Medical Social Worker. Graduation from an accredited School of Social Work with an MSW degree. Additional Requirements: functions independently prioritizes and organizes workload effectively capable of critical thinking and executing decisions makes recommendations to an interdisciplinary team builds trust and respect among peers and other departments participation as a team player has the knowledge and skill to work well with all inpatient populations.
