Job Abstract

PsychPros, Inc. has a Medical Social Worker assignment available with full-service hospitals in Fontana, CA, and Ontario, CA. The Social Worker will assist patients and their families with care planning and discharge plans, complete psychosocial assessments, and partner with patients to identify needs and develop and implement individual treatment plans based on mutually agreed-upon treatment plans. In addition, SW will also discuss options for care proactively including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate.

This is a temporary employment assignment expected to last 3 months and pays $35.00 an hour. The schedule is Mon-Fri 8:30 am-5 pm.

Qualifications:
Master's degree in social work with an active BBS registration as an ACSW/ASW and one (1) year of experience in a medical/health care setting. Hospital acute care experience is required. Demonstrated ability to work on a multidisciplinary team. Knowledge of chronic and acute disease and how it impacts patient and family functioning. Must be computer-literate and, preferably, experienced in automated clinical information systems.
