- California Speech-Language Pathology Services Credential OR California Clinical Rehabilitative Services Credential
- If currently enrolled in a Speech-Language Pathology graduate program, please include proof of enrollment letter from the university or copy of official/unofficial transcripts showing current enrollment in the program in lieu of a California SLP Credential
