The Nurse Practitioner (NP) provides quality patient care in variety of settings in collaboration with and under the direction of the Hospice and Palliative Medicine Physician. Follows established nursing and organizational standards, procedures, and practice. Gives specific patient care directions to
nursing and other staff. Provides education and support for patients, families and staff members.
Quality Provision of Services:
Provides direct patient care to patients with a progressive or terminal illness, evaluates outcomes, consults with the Physician, nursing staff and other members of the Interdisciplinary team as indicated to ensure optimal patient care in an effective and efficient manner. Performs examinations, procedures, and other processes related to direct patient care. Performs initial history and physical, assesses patient’s condition, initiates and individualizes standing orders and writes additional orders as necessary and in collaborating with nursing staff, develops individual plan of care. Makes daily rounds on patients, assesses patients’ conditions and writes routine orders with consultation and countersignature of the Physician. Writes and/or dictates appropriate notes in the medical record to be reviewed and countersigned by the Physician, or on many occasions, independently. Performs advanced assessments of end-of-life issues including pain; SO; N/V; constipation; wounds; anorexia; weakness; psycho-social; emotional/spiritual issues, etc. In collaboration with the nursing, develops individualized education plans for patient/family education, based on patient/family identified needs. Provides patient and family education as needed to maintain quality care, to decrease patient and family anxiety, to facilitate the patient living until death and to assure both the patient and the family have the skills to care for the patient. Participates in discharge planning including follow-up care decisions, writing prescriptions, writing discharge orders and instructions, etc.
People/Communications:
Participates as a member of the interdisciplinary team to develop and implement a patient directed individualized plan of care and to assure the availability of information to team members. Provides leadership and coordination for the interdisciplinary team in the absence of the Clinical Supervisor. Provides teaching to staff on an as needed basis to enhance care of the patient/family. Functions as a role model in providing exceptional customer service to patients and families by effective and timely communication.
Organizational/Regulatory Compliance:
Documents in appropriate chart and forms in an accurate and timely manner to provide a written record of assessments, plan of care and to assure the availability of information to team members. Maintains established departmental policies and procedures; objectives; quality improvement program; safety; environmental and infection control standards. Keeps knowledge base and skill levels current to assure quality patient care and professional growth.
Financial Stewardship:
Achieves expected productivity standard (based on patient visits and caseload) as defined by the organization and in alignment with best practices.
