Position Overview:
Provides direct patient care utilizing the nursing process under the direction and supervision of the Registered Nurse.
Reviews diagnostic test results and reports abnormal results and changes in patient condition to the Registered Nurse, physician and appropriate healthcare team members in a timely manner.
Collaborates with the Registered Nurse on development and implementation of the patients plan of care.
Administers prescribed medications, therapies and treatments in accordance with Licensed Practical Nursing Standards.
Maintains and reviews patient medical records, notes, and physician orders. Documents all patient care activities in accordance with documentation standards.
Assists physicians, Registered Nurse and multidisciplinary team with data collection, planning, intervention and evaluation of patient and family care to achieve desired outcomes.
Implements components of the plan of care with the Licensed Practical Nurse scope of practice and competency to promote desired outcomes.
Assists with discharge planning and carrying out the patients teaching plan under the direction of a Registered Nurse.
Evaluates and maintains the environment to promote patient and healthcare team safety.
Collaborates with the Registered Nurse and multidisciplinary team to advocate for the patient and supports the patient's psychological, ethical, spiritual and cultural needs.
Provides compassionate basic nursing care; maintains patient privacy and confidentiality.
Communicates effectively and works collaboratively within the interdisciplinary team. Promotes team building to foster a positive work environment. Effectively communicates with the multidisciplinary team members, patients and families to achieve desired team goals. Seeks out appropriate resources to increase practice knowledge.
Participates on unit-based committees and regularly attends unit staff meetings and unit-based educational offerings. Works with team members to achieve maximum productivity. Demonstrates willingness to assist others.

MINIMUM REQUIREMENTS:
Education: Graduate of an accredited Licensed Practical School of Nursing Program.
Experience: Less than one year related experience.
Other Credentials: AHA BLS - Healthcare Provider, Licensed Practical Nurse
Knowledge and Skills: Proficient in English, verbal and written communication skills; computer skills.
Occasional physical demands include: Sitting , Climbing (e.g., stairs or ladders) , Squat/kneel/crawl , Wrist position deviation , Taste or Smell
