We are Now Hiring a Qualified Intellectual Disability Professional in Brooklyn, NY to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities. The Qualified Intellectual Disability Professional assists the Executive Director in developing and maintaining individual service plans that meet the needs of people with disabilities and state and area program guidelines.
Starting pay is $23.00/hour
Responsibilities
Develop individual service plans (ISP)/Person Centered Plans (PCP)
Coordinate admission and discharge activities of participants
Act as a liaison for area programs with clinical issues
Provide support and supervision to Sr. Community Support Staff regarding compliance and clinical guidelines
Prepare monthly goal progress notes and billing justification forms
Maintain and analyze all incident/accident reports
Facilitate selection of clinical consultants, and monitor their activity
Coordinate behavioral intervention plans, to include routine and emergency services
Monitor medication procedures, administration, and documentation
Evaluate participant satisfaction
Monitor staff compliance with plan implementation and licensing compliance
Evaluate staff performance
Employee must cooperate with the licensee and department staff in any inspection, inquiry or investigation
Additional tasks and responsibilities may be assigned
Minimum Requirements
Bachelor’s degree and two years’ experience in implementing community-based support service for people with disabilities
Valid driver’s license and good driving record
QIDP qualified
Excellent organizational skills, and the ability to set priorities and follow through under time constraints
Ability to effectively and professionally communicate with program participants, and other agencies, staff and members of the community
Training in Gentle Teaching methods
Ability to respond to crisis situations, exercise professional judgement, and use common sense
Knowledge of required state operating and reporting guidelines under OPWDD
Working Conditions
Work in residential programs, day programs, and in the community
Schedule may change due to business needs and may include evening and weekend hours
May be required to be on-call in cooperation with other management staff
Frequent lifting, stretching, and other physical exertion may be required
May be required to transport individuals utilizing your own vehicle or company provided vehicles
May be required to lift or move 25+ pounds
May assist with wheelchair transfer of non-ambulatory individuals
May be exposed to various medical conditions and communicable diseases
While performing the duties of the job, employee travels by automobile, utilizing own vehicle, and is exposed to changing weather conditions.
