FLEXIBLE SCHEDULE: Four 10-hour days or Five 8-hour days

TRANSPORTATION: Uber provided at company expense within Queens

NO NIGHTS, WEEKENDS, OR HOLIDAYS

• Location: Covering Queens, NY
• Transportation: Uber access to patients once in the borough (if you live in Queens, you can take Uber from home to the first visit and back home from the last visit)
• Role Distribution: 25% management, 75% clinical/patient-facing (home visits)
• Patient Care: Combination of high acuity patients that need to be seen once a month, sick/urgent visits, and post-hospitalization visits
• Non-Traditional RN Case Manager Role: No routine skilled nursing tasks expected apart from as-needed phlebotomy, wound care for educational purposes, and potential as-needed catheterization; we routinely refer to other services for ongoing/long-term skilled nursing

Typical Visit: Often shared with our Palliative Care Nurse Practitioner, including routine nursing assessments (vital signs, focused palliative review of systems, physical exams, goals of care conversations)

Attractive Pay

• Salary: $118,000-$125,000 annually
