Qualifications
- Education
  - Required: BSN or MSN or enrollment in an RN-BSN or RN-MSN program within 1 year of hire and completion within 5 years of hire if hired after July 1, 2012
- Licensure/Certification
  - Required: California R.N. license, Certified Diabetes Care and Education Specialist (CDCES) or will obtain CDCES certification within 12 months of hire or earlier if eligibility requirements have been met.
- Experience
  - Required: 2 years teaching or nursing
  - Preferred: Teaching in an acute hospital or medical environment
Essential Responsibilities
- Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
- Completes an individualized, initial assessment that includes relevant medical history, diabetes history and social history.
- Completes a written pre-program assessment of client’s education knowledge, self-management skills and diabetes related behaviors on the content areas for national standards
- Provides an education plan, which includes measurable learning objectives, and a client selected behavioral objective as applicable based on the assessments.
- Instructs clients in general diabetes education and /or self-blood glucose monitoring at group classes and/or at individual visit per MD order and according to ADA.
- Documents educational interventions/reinforcements on the education record.
- Writes progress notes of client instruction for the referral physician documenting the client’s comprehension
- Contacts physicians as needed to relay client information and/or report blood glucose levels over 300mg/dl.
- Provides contact & literature to nursing staff and patients when consulted for persons hospitalized with diabetes
- Functions as part of the Diabetes Team by interacting with other members in both education and clinical situations to promote to promote quality outpatient client services.
- Provides follow up education as needed per program guidelines
- Provides community outreach as needed
- Participates in the planning, promotion and evaluation of the CQI process
- Participates in product evaluation, i.e. client trials, new meters as needed
- Develops and /or updates educational tools for teaching.
- Performs other duties as assigned.
