Key Responsibilities:
- Mentor and oversee the work of Social Workers and Case Managers.
- Direct and ensure continuity of care and discharge planning to the least restrictive setting.
- Ensure compliance with Medicare, Medicaid, and managed care guidelines.
- Identify high-risk patients and provide social work services and complex discharge planning.
- Educate patients and families about discharge planning and eligibility for programs and services.
- Monitor discharge planning delays and identify trends for performance improvement.
Qualifications:
- Registered Nurse (RN)
- Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
- Certification in case management (CCM) preferred.
- Experience managing RN Case Managers and Social Workers.
