Partner with a highly collaborative, multidisciplinary team that shares your passion for serving America’s most vulnerable seniors. Our mission-driven teams include a physician, a nurse case manager, a community health worker, a social care manager, and a scheduling specialist. Their combined superpowers magnify your ability to uncover & address each patient’s unique clinical, social & logistical needs.
Develop trusting long-term patient/provider relationships that truly transform patients’ lives & empower you to do the best work of your career rather than the high volume, transactional “bandaid” approach expected at other healthcare organizations.
Responsibilities will include:
Build a complex panel of geriatric patients that is co-managed with patient’s PCP and specialists
Develop patient-centric plans of care and work within a multidisciplinary team to make patient’s goals a reality
Meet patients where they are through a hybrid of in-home and telehealth visits

You are comfortable working with members over video and phone, but also in-person in their homes

Finally and most importantly, you have a passion for making healthcare better, solving complex problems, and supporting the delivery of healthcare that we would want for our own family members
Required skills and experience:
An unrestricted and active APRN licensure to practice in the assigned geography
Current certification in your area of APRN specialization
Experience in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care
Desired skills and experience
Prior home based practice is a plus
Three or more years working as an APRN
Team player mentality with a can-do attitude
Extra interest in bilingual Spanish or Haitian Creole speakers or strong cultural competencies across a range of cultures
Salary range: $110,000-$125,000/year

Our ranges are purposefully broad to allow for growth within the role over time. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
