The Case Management RN Director will play a critical role in coordinating patient care to ensure that patients receive the appropriate services, resources, and support throughout their hospital stay and through their transition to other care settings. The Case Management RN Director will have the opportunity to utilize a wide range of skills, combining clinical, administrative, and social work expertise. Additionally, the Nurse Director Case Management will creatively negotiate, coordinate, and advocate for community based services (DYFS, APS, Office of Public Guardian, DDD, etc.) to assure the continuation of care and special services appropriate to the age of patients and the needs identified.

The Case Management Nurse Director will oversee Social Workers and RN Case Managers, ensuring that they are completing their PASRR Level 1 and EARC-PAS screens, while assisting the Physician Advisor in preparation of the Denial P2P and first level of appeals. The Case Management RN Director will identify and allocate appropriate resources to meet patient needs, including medical equipment, home care services, and community resources. This will require the Nurse Director Case Management to maintain current community information, and to establish community liaison relationships to maintain an open and efficient referral system, ensuring better service delivery to patients and their families.
