Job Description:

Overview: We are seeking two compassionate and experienced Nurse Practitioners to join our healthcare team on a per diem basis, focusing on house call visits in Detroit, Michigan. As a Per Diem House Call Nurse Practitioner, you will have a unique opportunity to deliver personalized healthcare services directly to patients in the comfort of their homes, ensuring a holistic approach to their well-being.

Responsibilities:

1. Conduct comprehensive assessments during home visits to evaluate the health status of patients.
2. Develop and implement individualized care plans that address the unique needs and preferences of each patient.
3. Perform diagnostic tests, order appropriate laboratory work, and interpret results to inform treatment decisions.
4. Provide patient and family education on health management, disease prevention, and lifestyle modifications.
5. Collaborate with primary care physicians and other healthcare professionals to coordinate and optimize patient care.
6. Prescribe medications and treatments as needed and ensure adherence to established care plans.
7. Participate in regular case conferences and meetings with the interdisciplinary healthcare team to discuss patient progress and coordinate care.
8. Maintain accurate and up-to-date electronic health records (EHR) for all house call visits.
9. Demonstrate cultural competence and sensitivity when interacting with diverse patient populations.
10. Adhere to all regulatory requirements, ethical standards, and legal guidelines in the provision of healthcare services.

Qualifications:

1. Master's or Doctorate degree in Nursing from an accredited institution.
2. Current and unrestricted Nurse Practitioner license in the state of Michigan.
3. Certification as a Nurse Practitioner in a relevant specialty (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner).
4. Minimum of 4 years of clinical experience as a Nurse Practitioner, with a preference for experience in home-based care or geriatrics.
5. Strong communication and interpersonal skills to engage effectively with patients, families, and caregivers.
6. Ability to work independently and demonstrate sound clinical judgment during home visits.
7. Proficiency in using electronic health records (EHR) and other healthcare-related software.
8. Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
